Fayette Community Services is a County Government Agency which was established to provide funding for services to persons with Mental Illness, Mental Retardation and Developmental Disabilities. The Community Service Office includes the Central Point of Coordination and the Case management Unit. The Community Service office assists individuals with disabilities in accessing community based services to meet their specific needs. We are dedicated to maximizing individual's self-determination and independence within the community network.
- Central Point of Coordination
The CPC is the single point of entry to receiving services funded by the County. To determine if you are eligible for county funding you must apply to the Community Service office located in the Fayette County Courthouse. You will be asked to complete and sign a CPC application and sign necessary releases of information to obtain verification of legal settlement, diagnosis and financial eligibility. A noti8ce of decision will be mailed to you within 10 working days of receiving the completed CPC application. If approved you will be referred to the County Social worker or Case Management to assist you in coordinating you services.
To qualify for County funded services you must be 18 years of age or older, have an established diagnosis of mental illness, mental retardation, or developmental disability. Legal settlement, which refers to a legal concept of determining which county is legally responsible for the cost of the service requested, will be established. Residency is not necessarily6 legal settlement. Financial eligibility will also be determined by the CPC.

- Title XIX Case Management
Fayette County Case Management is a Medicaid service that assists individuals in identifying, developing and accessing community resources and supports, that are needed to accomplish desired goals. The Case management unit is located in the Fayette County Courthouse. Case Management serves persons 18 years of age or older, with a primary diagnosis of mental retardation, mental illness or developmental disability and are eligible for Title XIX. Children are also served if they qualify for the MR/Waiver services and are referred through DHS.
To apply for Fayette County Targeted Title XIX Case Management contact the Community Services office and request Case Management. A Case Manager will then contact you within five working days to set up a meeting to complete an intake packet. After eligibility has been verified, the Case manager will send out a Notice of Decision and meet with you again to complete a social history, an assessment, coordinate services, obtain County funding and establish a comprehensive plan for the team to follow.
